UC Human Resources recently launched a systemwide project (http://atyourservice.ucop.edu/family-member-verification/) to verify the eligibility of all faculty, staff and retiree family members enrolled in UC health benefits.
· UC is requiring all faculty, staff and retirees who have enrolled family members in their medical, dental and/or vision insurance plan to provide documents to verify their family members are eligible for coverage.
· If you had a family member enrolled in a health benefit plan as of February 13, 2012 you should have received a packet of information in the mail that includes the names of enrolled family members, a list of the documents that must be submitted for each person listed and instructions on how to submit the documents. You must respond by May 17, 2012.
· If you have previously provided documentation to verify your family members’ eligibility you must respond to this request as well.
· The verification project is being undertaken by UC in order to identify ineligible people who are currently enrolled in UC health plans. As long as you respond to the information packet in good faith, there will be no penalty for having had ineligible members enrolled.
If you do not respond by May 17th, you may risk your own de-enrollment from UC insurance as well as de-enrollment of your family members. If you do respond but do not provide sufficient documentation, you also may risk having your family members de-enrolled.
· If you submit documents such as tax returns or credit card statements, be sure to black out any personal information (social security numbers, financial data, etc.) not needed for the eligibility verification.
· Because of the size of this project—more than 175,000 family members must be verified—UC has hired an outside vendor, Secova, to administer the project. UC chose Secova because of their experience in family member verification projects for large organizations, their commitment to customer service and their state-of-the art security processes to protect the confidential data of our employees and retirees.
· Questions, including concerns regarding the type and timing of documentation, should be directed to Secova at 1-877-632-8126 (toll-free). Representatives are available 24 hours a day, 7 days a week. Secova is able to provide assistance in multiple languages.

Debe responder no más tarde que el 17 de mayo de 2012.
· Si anteriormente ha provisto documentos para verificar la elegibilidad de sus familiares, también debe responder a este pedido.
· Si no responde a llegar el 17 de mayo, corre el riesgo de que se le anule su inscripción en el seguro de UC, además de la de sus familiares. Si efectivamente responde pero no presenta suficientes documentos, puede correr el riesgo de que se le anule la inscripción de sus familiares.
