Daily symptom check guidance for employees

Effective Aug. 1, the daily symptom check questionnaire will no longer be required for employees working on-site at UCSC-owned or leased facilities. Instead, only those employees who have possible COVID symptoms should complete the daily symptom check questionnaire.

All employees should continue to self-assess your health status and possible COVID symptoms each day before coming on site. If you experience any COVID-like symptoms, please stay home, complete the symptom check questionnaire, and take an over-the-counter COVID test (you can order eight free COVID tests through the United States Postal Service at this link). If you receive a positive result, please report your results to your manager or supervisor and contact your medical provider. You may be eligible for COVID-19 treatment even if your symptoms are mild. 

The daily symptom check questionnaire will no longer be emailed to enrolled employees each day. You can access the symptom check questionnaire directly at the Slug Strong website “Checking for Symptoms” page. Please also check out our FAQs for more information.

Supervisors and managers should share this information and expectations with their employees. If an employee reports a positive test result, notify check@ucsc.edu

As a reminder, masking is strongly recommended in all indoor campus settings, and COVID testing remains available on campus.
